By providing personal care, memory assistance, companionship and any other services necessary, we can help support the senior who is suffering and their family as well.

Are you or a loved one suffering from a cognitive impairment? We know all too well that managing these situations is never easy. Not only do you have to accept the changes you are going through on a daily basis, but you have to manage the symptoms and find caregivers who have the skill and compassion to help you through such a trying time.
The goal of our program is to engage with the cognitive abilities of our patients in a way that is meaningful and helpful to every one of our clients.
The Cognitive Caregivers we provide receive intense training on cognitive health, dementia, Alzheimer’s and other issues, because we want them to provide the best care possible. We also provide sensitivity training to ensure our caregivers can support patients in a caring and compassionate way. We also have nurses who are experts at Cognitive Simulation Therapy.
The Cognitive Care Program we offer includes:
A Professional Health and Lifestyle Assessment is completed, which will serve as the baseline for the level of care you require.
- Ongoing Care
Based on the medical needs of the client, along with their requests, a medical care plan is created. Cognitive Caregivers will provide the various home and health care services a client requires.
A follow-up session always takes place, where a registered nurse and Cognitive Caregiver comes ot your home in order to talk with the client and to make sure all their needs are being met.
